If OFW / Seaman :
1. Certificate of Employment or Contract
2. Special Power Of Attorney (Notarized by Consul)
2. Proof of Remittances / 3 mos. Bank Statement
3. Must be Resident in M. Mla/Bulacan/Cavite/Laguna/Rizal
If Own Business / Self-Employed :
1. Business Registration
2. Latest Income Tax Return
3. 3 mos. Latest Bank Statement
4. Must be Resident in M. Mla/Bulacan/Cavite/Laguna/Rizal
